Appointment and Financial Policy
Thank you for choosing us as your dental care provider. We are committed to providing you with the best possible care. The following is our appointment and financial policy. Please read and sign below.
Payment for services are due at the time of services, unless prior arrangements have been made with any of U smile family dentistry staff members.
- We accept Visa, Master card, check, or cash for payment of the amount due. Payment plans are available; please ask for details if you are interested.
- For unaccompanied minors, we ask that financial arrangements be made prior to the appointment day.
- Dental Insurance
Our office understands the value of insurance benefits to our patients. We will file your insurance as a courtesy to you and supply all documentation to assure your maximum benefits. We will estimate your insurance coverage and estimate your portion for your treatment, which is due on the date of service. As this is only an estimate, you may have co-pays at the time of your office visit due, if there is any overpaid co-payments we will gladly issue you a refund after we receive full payment from your dental insurance. In a situation where your dental insurance does not cover your dental treatment, you will be responsible for full payment for any treatment rendered by your doctor at U smile family dentistry.
- Missed Appointments
Appointments are valuable blocks of time. Please help us serve you and all our patients best by keeping your scheduled appointment. Please give us a 48 hours notice if you are unable to keep your appointment. Please be aware that by missing 3 appointments with a no call/no show, or failure to notify our staff of any appointment changes, 3 cancelled appointments without a 48 hour prior notice may be subject to dismissal from our office.
- Returned Checks
There will be a $35 dollars fee for all returned or stopped checks. This fee covers the processing fees that are charged to our office.
We would be happy to discuss any questions or concerns that you may have about your appointments, treatments or any financial concerns you may have. We also like to address that if you feel that you may not be able to make any payments on time, that you notify the office immediately to make any necessary arrangements to your account.
